城乡医保统筹如何影响收入差距———融合促健防贫与普惠城乡双路径的分析

摘要:

本文基于促健防贫普惠城乡的双视角使用中国家庭追踪调查数据运用多期双重差分模型探讨城乡医保统筹政策与居民收入差距的内在逻辑关联ꎮ 研究表明统筹政策能够显著缩小居民收入差距、提升居民健康与医保待遇偿付水平并对低收入群体发挥了关键的兜底扶贫作用特别是在减轻罹患重疾的农村居民医疗负担、提升医保偿付能力方面成效尤为突出基于此应继续优化城乡医保统筹政策重点加强针对困难群体的医保待遇切实减轻其医疗经济负担增强医保制度的可及性与公平性

Abstract:

This article focuses on narrowing the income gap among residents, based on the dual perspectives of "promoting health and poverty prevention" and "inclusive urban-rural development". Using Chinese household tracking survey data and a multi–period difference-in-differences model, it explores the inherent logical relationship between the urban- rural medical insurance coordination policy and the income gap among residents. The study found that the coordinated urban–rural medical insurance policy can significantly narrow the income gap among residents, and this conclusion still holds true after a series of robustness tests. Compared with the New Rural Cooperative Medical Scheme and Urban Employee Basie Medical Insurance, the coordinated medical insurance has a positive effect in promoting horizontal equity and vertical public equity. The results of the mechanism test prove that the coordinated policy has significantly improved the level of residents' health and medical insurance benefits, and played a key role in poverty alleviation for low-income groups, especially in reducing the medical burden of rural residents suffering from serious diseases and improving their ability to pay for medical insurance. The effectiveness is particularly outstanding. Based on this, we should continue to optimize the urban-rural medical insurance coordination policy, focus on strengthening the medical insurance benefits for disadvantaged groups, effectively reduce their medical economic burden, and enhance the accessibility and fairness of the medical insurance system.
